What should I bring to my appointment?
Please bring the following to each clinic visit:
- Photo ID
- Your current health insurance card
- Your current address and phone number
- A list of the medications you are currently taking or the medication bottles of the prescriptions you currently have
- Any forms that you need your provider to complete for you

Yes, you can become a patient at our after a meet and greet appointment with one of our Family Doctor.
Who can book appointments?
Any patient can book an appointment at any time. All patients have the option to either walk-in or book an appointment.

- Routine visit to their Family Doctor
- If Family Doctor requests a specific follow up visit
- Pregnancy care
How does the walk-in service work?
Patients simply walk into the clinic without an appointment on a first-come first-serve basis, simply fill out the registration form and provide required HealthCare card and Photo ID. Depending on the number of walk in patients and the number of scheduled appointments ahead of you, wait times may vary. Wait times usually range from 5 minutes to 1 hours. Patients with scheduled appointments do not need to fill out registration form when they arrive if all required information was obtained at the time the appointment was booked, but instead should check in 5 minutes before their appointment times to be seen by the doctor.
How will I receive my test results?
We will contact you by phone with any significant abnormal results or as requested by the family Doctor. You may be asked to book a follow-up appointment. However, if you are interested in following up for a result, you can return to the clinic to discuss it at any time.
Can I get my test results over the phone?
In general, most test results will require a follow up visit to discuss them in the context of your medical condition. Sometimes the doctor needs to re-evaluate your symptoms. The receptionists are not trained to interpret lab findings, but they can sometimes help by telling you whether the results are back yet or not.
When to proceed to the Emergency Room?
If you are visiting our clinic and feel that your issue is so pressing that you cannot wait in line or should be seen by the doctor right away ahead of others, you should proceed to the ER.
Remember, our clinic receptionists are not Triage Nurses and are not expected to put one patient ahead of another based on priority. Rather, they will call you on a first come- first serve basis.
